Make Your Donation
Start by making your $50 minimum donation using the form on this page. It’s important to use this form to your right which triggers the process of requesting your DMV PIN. The name and address entered with your donation must match your vehicle registration exactly to avoid delays.
Your donation is fully tax-deductible, and 100% supports the Epilepsy Foundation’s programs. Online donations are preferred. If you would like to donate by check, please contact us for instructions.
Please note: The DMV will also charge a one-time $50 specialty plate fee, in addition to standard vehicle registration costs and any county-specific taxes.

Receive Your Certificate and PIN
Once your donation is received, we will email you a certificate with your unique PIN.
This PIN is required to obtain your plates through the Colorado DMV, either in person or online.
Please allow up to one week to receive your certificate and PIN. Be sure to wait until you have this before visiting the DMV.

With your PIN in hand, you’re ready to move forward.
The next steps depend on your situation.
If you’re purchasing a new vehicle, you can request your Epilepsy Foundation plates as part of your registration.
Once you have your certificate and PIN, follow our step-by-step guide to complete the process through the Colorado DMV. We’ve included screenshots and clear instructions so you know exactly what to expect at each step.

- Set aside a few minutes to walk through the DMV process
Ready to get started?
Go to https://mydmv.colorado.gov/_/ and click on Vehicle Services. Then click on Submit a first-time vehicle registration.
Once you have made your donation and you receive the email with your PIN, follow the instructions below to purchase your Epilepsy Awareness plates online. You can also purchase your plates in person at DMV offices around the state.
