You can take your support on the road with you. The official Epilepsy Foundation license plate is available in Colorado whether you’re purchasing a new vehicle or swapping out your current plates. It’s a simple way to show your support and make an impact every time you drive.

With a minimum one-time donation of $50, you can get your plate and help fund programs that support individuals and families affected by epilepsy. Even better, 100% of your donation goes directly to the Foundation.

  • You must use the form on the instructions page to make your $50 minimum donation. This ensures your contribution is correctly tied to your license plate request, not a general donation.
  • After your donation is submitted, please allow up to one week to receive your PIN. This PIN is required by the DMV to issue your plates.
  • Be sure to plan ahead. Your PIN is not issued instantly and cannot be provided while you are at the DMV.

How do I get my plates?

Make a minimum $50 one-time donation to the Epilepsy Foundation of Colorado & Wyoming using the form on this page. Donations must be specific to the license plate program and may not be combined with other donations.

Each plate set requires a separate donation (per vehicle). Please note that the DMV will assess a $50 fee for specialty plates, plus any county-specific taxes.

How do I order my plates?

After registering, take the PIN we email to you (please allow up to one week to receive the PIN) to the Colorado Department of Motor Vehicle Office to purchase your license plates. You can also purchase plates online. Here are step by step directions for renewing plates or for getting new plates on the mydmv.colorado.gov website.

Will I be required to make a donation every year when I re-register my car?

No. The donation is a one-time requirement.

Can I get plates for my motorcycle?

Yes. The steps and the cost are the same.

Can I make my license plate donation at the Motor Vehicle Office?

No. All license plate donations must be made here first.

My vehicle registration does not expire for several months, do I have to wait to get my license plates?

No, you can take your PIN to your Colorado County Motor Vehicle office any time. The DMV will issue a credit for any unused months left on your registration.​
