Make a minimum $50 one-time donation to the Epilepsy Foundation of Colorado & Wyoming using the form on this page. Donations must be specific to the license plate program and may not be combined with other donations.
Each plate set requires a separate donation (per vehicle). Please note that the DMV will assess a $50 fee for specialty plates, plus any county-specific taxes.
After registering, take the PIN we email to you (please allow up to one week to receive the PIN) to the Colorado Department of Motor Vehicle Office to purchase your license plates.
